Medical Records Department (MRD) located at 7th Floor of Sunway Medical Centre Velocity (SMCV) provides service for the request of Insurance Claim (pay and claim) and Medical Report from lawyers’ office, EPF, SOCSO or police reports.
The medical reports can be applied by walk in to MRD or via email to [email protected]
Operation hours for MRD:
Monday – Friday : 8.30am – 5.00pm
Saturday : 8.30am – 1.00pm
Sunday & Public holidays : Closed
Patients’ written consent is compulsory for the release of the medical information. For patients below 18 years old, the consent of a parent or legally acceptable representative is required. If the patient is deceased / incompetent, consent is required from the next of kin of the patient or the legally acceptable representative appointed by the patient/family members of the patient.
